Discworld MUD
A genuinely beloved game with a decent population and thirty-plus years of development across a world of more than a million rooms. But its accessibility is incidental — it is text, and text is readable, and that is as far as it goes. No documented visually-impaired programme, and heavy ASCII map use is a known friction class.
